Wednesday, December 13, 2017

CareMore Health will increase its provider collaboration network under an expanded agreement with MemorialCare effective January 1, 2018. Individuals enrolled in specific Anthem Blue Cross Medicare Advantage plans that receive their health care through CareMore Health will have access to MemorialCare physicians and medical centers located throughout Southern Los Angeles County and Orange County.

“We look forward to this expanded relationship with MemorialCare that will give our patients access to an elite hospital system in their area,” says Karen Schulte, Chief Operating Officer for CareMore Health. “This collaboration will further align two organizations that share a commitment to providing exceptional integrated care to patients.”

CareMore Health began their affiliation with MemorialCare last year through an agreement with one of their top hospitals, Long Beach Medical Center. This new agreement will give CareMore Health patients access to more than 200 MemorialCare providers and the health system’s five hospitals, including Saddleback Medical Center, Orange Coast Medical Center, Miller Children’s & Women’s Hospital Long Beach, Long Beach Medical Center and Community Medical Center Long Beach. Over the years, MemorialCare has received numerous acknowledgments for its high-quality care, including U.S. News & World Report’s list of Best Hospitals and America’s 50 Best Hospitals by Healthgrades.

“Providing patient-centered care has always been a priority for MemorialCare. We’re pleased to work with CareMore Health to provide patients in the area with the high-quality coordinated care that they need,” says Mark Schafer, MD, Chief Executive Officer for MemorialCare Medical Foundation.

Like MemorialCare, CareMore has built its model around surrounding patients with the coordinated care and services they need. Multidisciplinary teams of clinicians at the CareMore Care Centers collaborate to identify what it will take to keep patients physically and mentally healthy. CareMore’s effort to improve care for the frail, chronically ill and other underserved patients is part of its strategy to spend more time preventing and slowing the progression of disease.
