Conifer Health to Deliver Value-Based Care Services to Alameda Health

Conifer Health Solutions today announced that Alameda Health System (AHS) has engaged the company to provide Managed Care services to support HealthPAC, a comprehensive health access program for eligible enrollees of Alameda County, California. Conifer Health will provide managed care support services, financial risk management, population health management and nurse consulting services to help the four-hospital system better serve a population of 33,000 HealthPAC members.

Under terms of the agreement, Conifer Health will deliver health plan operations support including claims processing, referral and authorization, customer service and eligibility, among other services. The company will use its ConiferCore® solutions, including Outcomes Optimization and Population Health Intelligence, to enable AHS care managers and medical management professionals to proficiently manage the health of high-risk populations within HealthPAC. Conifer Health will also commence a consulting engagement to assess care management programs with the goal of improving the quality of care by aggregating, stratifying and then designing specific outreach programs for high-risk populations.

AHS chose Conifer Health because of its long-standing expertise in helping care delivery organizations assume and manage financial risk, specifically in the Medi-Cal market, where the growth of managed Medicaid organizations is projected for nearly every region in the state.
