Evolent Health Partners with Carilion Clinic to Enhance Care Delivery in Virginia

Evolent Health, a company providing an integrated value-based care platform to the nation's leading health systems and physician organizations, announced today its partnership with Carilion Clinic. The partnership will guide the development and execution of a value-based care strategy for improving the health of patients in Virginia.

Carilion Clinic is a tax-exempt health care organization headquartered in Roanoke, Virginia. With seven hospital facilities and a comprehensive primary and specialty care network comprised of more than 685 employed physicians across 70 specialties, Carilion provides quality care for nearly one million Virginians. The partnership between Evolent and Carilion will initially focus on implementing Evolent's proven model of care, including best-in-class care management programs, advanced patient stratification and analytics, and quality and network improvement.
