Centura Health and DaVita HealthCare Partners Introduce FullWell

Centura Health and DaVita HealthCare Partners unveiled FullWell, the brand name of the two partners’ recently announced joint venture, a company that aims to bring greater value to health care and lower costs for consumers in Colorado and Kansas.

“We are excited to announce the name of our joint effort and to begin to realize the vision of empowering physicians to succeed in the health care system of the future, where keeping communities well is rewarded,” said the company’s interim CEO, Martha Wofford. “With FullWell, we will provide the full array of tools to help people get and stay well.”

FullWell combines the expertise of the region’s leading health care system with a nationally recognized leader in physician-centric coordinated care to transform the way health care is delivered.

Specifically, FullWell will equip providers of Colorado Health Neighborhoods (CHN), the Centura Health physician-led network, with immediate access to data analytics. This data delivers critical information to support early identification of patients at high risk for chronic health conditions and real time feedback on the efficacy of treatments consumers may be receiving.

FullWell will employ tools to help engage, inform and empower consumers to actively manage and maintain their health through the trusting relationship with their provider. Additionally, FullWell brings access to improved information systems, team-based care coordination and financial incentives for better health outcomes to providers.
