In this collaboration, WVU Medicine nephrologists will integrate into the Interwell network, adopting its population health approach to kidney care. This comprehensive model includes a multidisciplinary team consisting of nurses, dietitians, social workers, and care coordinators. These professionals provide continuous support and education to patients, extending beyond traditional doctor visits to slow down disease progression.
Interwell employs predictive models to identify high-risk patients susceptible to disease advancement and hospitalizations. A specialized care team then offers prompt post-discharge outreach to prevent readmissions. By adopting a population health perspective, WVU Medicine aims to leverage technologies like machine learning to analyze disease progression data effectively and implement more targeted treatment plans.
